Immunization Records |

| The Department of Health and Hospitals, better known as the Health Department requires that children in group care be fully immunized to prevent disease. The following are a list of required immunizations as of June 2003. Hepatitis B Vaccine is required at: Birth – 2 mos – 12 mos Diphtheria, Tetanus, Pertussis (DTaP) 2 mos – 4 mos – 6 mos Haemophilus Influenzae (Hib) 2 mos – 4 mos – 6 mos with a booster at 15 mos and 4 years old Inactivated Polio (IPV) 2 mos – 4 mos – 12 mos with a booster at 4 years old Measles, Mumps and Rubella (MMR) 12 or 15 mos and again at 4 years old Varicella (chicken pox vaccine) 12 or 15 mos this is now a requirement for group care Pneumoccoccal (PCV) 2 mos – 4 mos – 6 mos – 12 mos ** We do recommend if your child is at high risk that you may want to ask your physician about the yearly flu vaccine.** |
