Who Is Eligible?

Student Health Service (SHS) is available to all currently registered UL Lafayette students who present a current school or pictured I.D.

How much will it cost?

Membership entitles you to medical evaluation with a clinician. This includes assessments for any illness, minor injuries and general physicals.

  • All registered UL Lafayette students are eligible for the use of Student Health Service with membership.
  • All students who are taking six (6) or more hours in the fall/spring and three (3) or more hours in the summer have been assessed a student fee at registration and are members.
  • Any student not meeting these hourly requirements may purchase a membership for $25.00 per semester.

All office procedures, laboratory work, pap smears, x-rays, referrals, prescriptions and injections are at the students expense. If you would like to know current prices for these services, please call (#48clinic) or come by the SHS.

Serious illnesses or injuries may be referred to a specialist or medical facility in the area. Should the clinician suggest referral to a medical specialist, the health services staff will help assist in making such arrangements.

Confidentiality Policy

The staff at Student Health Service follow the ethical guidelines of the medical profession and other medical specialties. Patient records are not a part of any student record. No information is released without written permission (a signed written consent) from the patient except as required by state law.
Students under the age of (18) eighteen are requested to have a written consent from their parent/guardian prior to treatment.

Policy on excuses for class

SHS will provide excuses only for students who have missed class while being seen in our clinic. Any extended excuse will be given at the discretion of the practitioner. Excuses given for a prolonged period of time will automatically be reported to the Dean of Students.

Policy on Chronic Medical Problems

Students with any chronic medical problem are to be under the care of their personal physician. SHS will work in collaboration with your physician to assist with blood work, x-ray, blood pressure readings, allergy injection administration, nebulizer treatments, and/or any other related procedure while you are attending UL Lafayette as long as your condition remains stable. Students with chronic medical problems can be assessed and treated at SHS for any other unrelated illness or injury.

Services not offered

Dental care, obstetrical care, and major surgical procedures are not available at SHS. Our staff can provide local referrals for these services.
