Who Is Eligible?

• All registered UL Lafayette students are eligible
  specifically:
• Fall registration eligibility will extend until the 1st day of the Spring semester.
• Spring registration eligibility will extend until the 1st day of the Summer semester.
• Summer eligibility may occur by the following options and will extend until the 1st day of the Fall semester:
   • Registered for Summer classes
   • Registered for previous Spring and upcoming Fall
         semester
       • Registered athletes required to be on campus before
         Fall semester begins

• Eligibility ends with graduation, except with the following
  circumstances, and are decided on an individual basis:
   • Continuation of an ongoing medical issue
   • General exam for the graduate to fulfill requirements
      needed for employment or continued education purposes

• Also, students who are enrolled in the Summer Upward
    Bound programs:
   • Non-bridge and Math & Science students that have a
     current medical consent and HIPAA acknowledgement
     form signed by a parent or guardian

 

Is There a Cost?

There is a membership fee that entitles you to a               medical evaluation with a clinician. This includes assess-    ments for any illness, minor injuries and general physicals.    • All students taking six (6) or more hours in the Fall or      Spring and three (3) or more hours in the Summer have      been assessed a student membership fee at registration      and are members.
   • Any student not meeting these hourly requirements may       purchase a membership for $25.00 per semester.
There are billable services which include all office
   procedures, lab work, pap smears, x-rays, referrals, IV
   therapy, prescriptions & injections and are at the expense    of the student. If you would like information on current

Tuesday Staff Meeting 7:30am-8:30am

Appointment Hours:
Monday, Wednesday & Thursday 8am-4pm
Tuesday 8:30am-4pm
Friday 8am-11:30am

Nurse Triage Only:
Monday-Thursday: 3:30pm-4:30pm
Friday: 11am-11:45am

As a response to our continuous surveys, we are offering a combination of appointments and walk-in services to lessen your waiting time, to utilize the SHS more efficiently throughout the day, and to continue to provide quality healthcare.

Appointments are scheduled for all women's services and minor procedures.

All patients are triaged by a nurse prior to being seen by a clinician.

All patients should:

  • Allow time for completing and/or updating personal data each semester.
  • Bring proof of insurance coverage other than the Student Basic Policy each semester, as insurance coverage may change over time and correct information allows for accurate billing.

SHS is closed on weekends and university holidays.
If you need assistance when the clinic is closed call University Police at 482-6447 or seek care at a local Walk In Clinic or Emergency Room.

Confidentiality Policy

UL Lafayette Student Health Service follows the ethical guidelines of the medical profession. Patient records are not a part of any student's academic record. No information is released without written permission (a signed written consent) from the patient except as required by state law.
Students under the age of (18) eighteen must have a written consent from their parent/guardian prior to treatment.


Policy on excuses for class

SHS will provide excuses only for students who have missed class while being seen in our clinic. Any extended excuse will be given at the discretion of the practitioner. Excuses given for a prolonged period of time will automatically be reported to the Dean of Students.

Policy on Chronic Medical Problems

Students with any chronic medical problem are to be under the care of their personal physician. SHS will work in collaboration with your physician to assist with blood work, x-ray, blood pressure readings, allergy injection administration, nebulizer treatments, and/or any other related procedure while you are attending UL Lafayette as long as your condition remains stable. Students with chronic medical problems can be assessed and treated at SHS for any other unrelated illness or injury.

Services not offered

Dental care, obstetrical care, and major surgical procedures are not available at SHS. In addition, motor vehicle accident related injuries, work related injuries, assaults involving litigation, orthopedic physicals, athletic physicals and immigration physicals are not seen at SHS. Our staff can provide local referrals for these services. The university does not provide transportation.
